When you see your primary care manager, please bring a current medication list with your appointment and also let your provider know what medication refills are needed. If you are a diabetic, bring a log of your blood sugars to your appointment. If you have a history of hypertension, bring a log of your blood pressures to your appointment. If you have not seen your provider in a year, please schedule an appointment to see your primary care manager.
